All information on the order form must be complete. Requests with incomplete information must be accompanied by a written explanation of omission. If any relevant information is left blank, the request will not be filled.
Eligibility Requirements
Eligibility to apply for a birth certificate includes:
(a) you, if the record pertains to your own birth and you are at least 13 years old
(b) parents of child
(c) an Agent or person with written authorization of (a) and (b) above
(d) guardian ( must have send guardianship papers with application )
