Are you trying to get an Ontario birth certificate?

Why do you need an Ontario birth certificate? An Ontario Birth Certificate is an important form of identification because it can be used to obtain other legal identification or documents.  Some employers require that you provide them a copy of your Ontario birth certificate.  It is also an important document to have if traveling or relocated to a foreign country.  Ontario birth certificates can only be issued for people born in Ontario, and are based on the information in the original Statement of Live Birth. 

Who can get an Ontario birth certificate? If you are at least 13 years of age, you can get your Ontario birth certificate.  If you are the parent named on a Child's Statement of Live Birth.  If you are the executor of the will of someone who has passed away.   

What is the easiest way getting an Ontario birth certificate?  The easiest way of getting an Ontario birth certificate is with the birth of a new child.  If you are a new parent the procedure for getting your newborn child an Ontario birth certificate is by completing the  registration of your child's birth.  At the time of your child's birth a permanent record called a Statement of Live Birth should have been issued to you by either the hospital or the midwife.   Always make sure to review the information on the Statement of Live Birth so that it is accurate.  The statement of live birth is not a birth certificate, it must be signed by both parents and returned to the government.  If the information about the father is not available then only the mother must sign the Statement of Life Birth.  The Statement of Live Birth must then be mailed to the Division Registrar, of the city the child was born in.  The Division Registrar is the department of a city government which will forward the paper work to the Office of the Register General, which is the provincial government, in this case that of the Ontario government, handling Ontario birth certificates.   Shortly after signing the Statement of Live Birth and sending it to the municipal government, you will receive a Notice of Birth Registration.  This is an acknowledgment that the government has recorded that your child has been born, only then can you as a parent apply for an Ontario Birth Certificate for your child.

Are you trying to get a replacement Ontario birth certificate?

  Many times people lose birth certificates and need a replacement one issued to them.  There are two types of birth certificates that one can get, the first is a small-size birth certificate which is wallet sized and can be used to carry around, the other is a certified birth certificate which is longer.  The short form has less information than the certified long form, the certified long form includes not only, name, date of birth, certificate number, birthplace, sex, date of registration, registration number, date issued, which the short form contains but also location of birth such as hospital or birthing centre, parents' names, parents' date of birth, mother's mailing address at the time of birth, duration of pregnancy, parents' birthplace, weight of child at birth, attendant at birth, type of birth (single, multiple, etc.) and number of children born to mother.  

What if my child requires a replacement Ontario birth certificate?  If your child is 9 years or older you will need to have a guarantor.   

What is a guarantor for when I apply for an Ontario birth certificate? 

A guarantor is a respected member of the community who knows you and if contacted can confirm your identity.  They would have to know you for two or more years.  You do not need for the guarantor to sign your form, but you will have to ask for their permission before you use their information on your form.  The information that you need about the guarantor is, their first and last name, their occupation with their work address and phone number.  Be advised that the guarantor has to be a Canadian citizen themselves and only some occupations qualify for being a guarantor.  

Does the guarantor, for my application to get a replacement Ontario Birth Certificate, need to be in a specific occupation?  The Ontario government requires that the guarantor be an upstanding citizen and typically recognizes several professions, because certain roles are difficult to acquire and have high standards, and must meet professional requirement.  The list of professions that can qualify as a guarantor is extensive but can be broken down easily.  Any citizen who holds public office such as a municipal mayor, member of parliament, or a Chief of a band recognized under the Indian Act can act as a guarantor; recognized teachers in either the University, College, High school, or elementary level; those involved in the medical community such as doctors, midwives, nurses, optometrists, pharmacists, psychologists, social workers, surgeons, or veterinarian; citizens that work in the field of justice, such as police officers be it municipal, provincial, or federal, judges, lawyers; members of a professional community such as engineers, certified accountants, senior administrator, even ministers of a religion authorized to perform marriages can be a guarantor.   There are many choices for people to act as your guarantor. 

What if I need my Ontario birth certificate quickly?   There are circumstances in which the government of Ontario will quicken the pace that an Ontario birth certificate is issued.  Several emergency reasons are acceptable, a medical emergency is always a valid reason for receiving expedited service, so is new employment which requires Ontario birth certificate documentation.  Other reasons that the Ontario government will expedite service for applicants are wedding plans if one  of the applicants is either the bride or groom, or an immigration hearing.  For all emergency situations  the applicant must have proof that indeed there is an emergency, proof could be airline tickets or travel reservations,  letter confirming new employment, or wedding reservations if you are either the bride or groom. 

Can my newborn travel without an Ontario birth certificate?  If you cannot obtain an Ontario birth certificate for your newborn before having to travel there is a solution.  The municipality your baby was born in can provide for you with a Temporary Confirmation of Birth Letter.  This letter is written by the municipality after you have submitted the Statement of Live Birth.  The municipality sends the Statement of Live Birth to the Ontario government but in the mean time it can issue a Temporary Confirmation of Birth Letter.  The Temporary Confirmation of Birth Letter can only be used for up to 90 days after you submitted your Statement of Live Birth document for your new born child.

What specific information is needed to obtain an Ontario birth certificate?  When obtaining an Ontario birth certificate there is some information which you should have on hand, your first name, last name and middle name if you have one,  as well as your current mailing address, your date of birth, gender, city of birth, your mother's maiden name, your father's last and first name, your Guarantor's first and last name as well as their occupation, and how long they have known you.  Aside from such crucial information it would be helpful if you could provide any other last names used by either of your parents or yourself, your parent's date of birth and place of their birth, your parents age at the time of your birth, your weight at birth, the location of your birth such as the name of the hospital, birthing centre, home, the attendant (doctor, midwife or other person present) at the birth .
