Eligibility Requirements
Eligibility to apply for a ALBERTA birth certificate includes:
(a) you: if the record pertains to your own birth
(b) parents of child
(c) an Agent or person with written authorization of (a) and (b) above
(d) guardian (must have send guardianship papers with application )
